Operative Treatment of Neck Fractures of the Femur


Abstract
we analysed twenty-five cases of femoral neck fractures, between the ages of sixteen and sixtynine years, who were treated with internal fixation and followed for more than one year at Chosun University Hospital, from January 1990 to June 1994,
and
following results were obtained. The most common type of fracture was transcervical in 16 cases(64%), Gardens grade 4 in 16 cases(64%) and Pauwels grade 3 in 18 cases(72%).
The methods of internal fixation were multiple pinning in 17 cases(68%), compresssion hip screw in 7 cases(28%) and compression hip screw with additional pinning in 1 case(48%), good 7cases(28%), fair in 4 cases(16%), and poor in 2 cases(8%).
There were complications in 7 cases(28%): avascular necrosis in 6 cases(24%) and nonunion 1 case(4%).
The time interval between the injury and the methods of treatment did not affect the results of the treatments, but initial displacement and execution of satisfactory reduction affect the results of the treatments. With the experience based on 25
cases,
we can conclude that the good results of femoral neck fracture patients were obtained by anatomic reduction and rigid internal fixation using multiple pin or compression hip screw.
